New Patient Registrationregister_now

Patients living within the practice area are welcome to register and our reception staff will be happy to to assist you in the process.

All new patients to the surgery are allocated a named GP and are informed of such. All existing patients on the register have been allocated and informed of their named GP. Patients can however book to see whichever GP they choose. The named GP is responsible for the patient's overall care. If you wish to know whom your named GP is please contact the surgery. If you have a preference the surgery will make reasonable efforts to accommodate this.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. We will also ask you to sign a 'contract of care' which sets out patient and doctor responsibilities and obligations.

Medical treatment is available from the date of registration.
